Is there a time frame in which a diagnosis should no longer be assigned to a condition? Example: Patient had a kidney stone in 2000, but nothing symptomatic since then. (He is being followed for CKD)
Should we still be coding the 592.0 as a condition or should it be changed to history-of (or not coded at all)?

Since the patient has not had a symptomatic stone since 2000, but is being treated for CKD I think it would be appropriate to code a hx of kidney stone as second dx behind the CKD that he is currently being treated for.
