If the HPI states "55 yr old female presents for hypertension, insomnia, and anxiety." what would these illnesses be considered? i know that they are chronic but it is not specified in the note how long the patient has had these nor a current status of these conditions. So would they just be classified as "acute, uncomplicate"?
 
if the length of condition status is not noted, I would not be able to be sure how diagnose new problem/s with out more details. Especially the HTN.
The insomnia and anxiety could be brand new issues and may be considered acute, uncomplicated but we can't really diagnose on speculation of interpretation
I would query the provider to be absolutely sure.
