I am having trouble deciding the right way to code this. The below information is all I have to go on.

Procedure: Condylomata Excision x 6: Injected lidocaine before procedure and applied silver nitrate after.
Vaginal Biopsy: at introitus bx of lesion concerning for dysplasia


Assessment: Condyloma, Vaginal Mass


The provider has listed an E/m along with the procedures. My main concern is finding the correct code for the condylomata excision. There are no measurements for these, I've checked with the provider and she confirmed none taken.

Any guidance is appreciated.
