Is this proper dictation for ROS or not? I am getting conflicting info that the systems must be listed and stated as negative to be acceptable if it is not pertinent to their presenting problem... Thanks!

Review of Systems
See HPI
The patient's last ROS was reviewed in it's entirety and has no changes unless noted in HPI.
 
For this to be acceptable it must be stated as to the exact date of the ROS and the location, and whether this was reviewed with the patient... such as the ROS from 6-10-2010 in the office chart was reviewed with the patient and no changes were noted.
