Conflicting Stages of CKD

If a doctor documents "CKD III Stable" in the active problem list and then in the assessment documents "CKD IV-V Stable", do you code unspecified CKD since there are conflicting stages or code the lowest stage or the highest stage?

I work for a health plan and do not have the ability to query the physician.
 
Unfortunate there is no viable solution here. This is probably an EHR error. You will need a copy of the entire note to see what is addressed in the exam. If it is still still not clear then you cannot code it without clarification.
