The only code for repair of congenital diaphragmatic hernia is 39503 and the code descriptor states neonatal. What code do you use if the patient is non-neonate, 7 month old infant? 39599 unlisted procedure diaphragm? I made several readings and research, some recommend 39599 and some still using 39503. It should be 39599 unlisted because there is no code that fits or meet the criteria based on the age of the patient.

While the congenital diaphragmatic hernia is usually repaired early in the neonatal period, there are codes for the non-traumatic paraesophageal hernia repair (43332-43337 for open approaches) that may describe the procedure performed. I did see a case report of a child who was diagnosed at age 4.
