This is a question that I had to answer on my, on-line Adv.Coding Class; This question had me upside down, any advice, on how this could have been presented differently?

A patient is admitted in decompensated congestive heart failure resulting from end stage cardiomyopathy. Laboratory findings are: Metabolic acidosis, elevated BUN/creatine and elevated liver enzymes. Physicians diagnosis is both renal and heart failure and shock liver.
