I am getting many letters from insurance companies asking for more detailed notes on the conscious sedation, but we include everything in a list format like time, drug, duration, who administrated, any complications, etc and they still keep coming back saying not enough. Am I missing something? I have never gotten anywhere productive on the phone with a rep because they are just reading from a screen. Is there a helpful template or example? I would be very grateful for any guidance!
