I have a piece of a note here for an ED visit that I'm auditing. In both 95 and 97g, how would you count this statement?

General: Has slightly hoarse voice. Alert & oriented, nontoxic, well hydrated, well appearing in no acute distress, speaking full sentences on RA

97g-
The "Alert & oriented, nontoxic, well hydrated, well appearing in no acute distress" I'm counting as general appearance under Constitutional.

"Speaking full sentences on RA" - would that get credit for "Communication Assessment" in constitutional in the ENT specialty exam?

95g -
I would count the whole statement as Constitutional.
