Dr. did a consult in the hospital and no one followed the patient for 3 weeks and the hospital called again for another consult. Can we bill this as a consult or do we have to code it different?

Not sure I understand what "no one followed the patient for 3 weeks" means but to answer your question...yes...If your provider receives another consult request for the intention of rendering another opinion, it is billable. ;)
 
Instructions for inpatient consultations codes state "only one consultation should be reported by a consultant per admission". So if this is the same hospitalization, I believe you should bill 99231-99233.
