Can the PCP bill for a consultation when seeing the patient for a surgical clearance?
 
Yes, a consultation is the request for an opinion, only.

I have seen a physician do a consultation on the same problem more than once, the request came from 2 or more different physicians.

A consultation is not new or established driven. Just pay attention to your payor rules as more and more insurances are not recognizing them any longer, they are following in MCR's footsteps.
