Wiki Consultations within a shared medical record

I know in order to bill for a consult, we have to have the request, reason, and report back to. But if we have a shared medical record and patient comes in for a consult and the provider does not mention the request but we find the order from the requesting.. is that still sufficient for billing a consult?
