When there are multiple gestations, and twin B is a spontaneous abortion- I've had mixed findings of what the 7th digit is that we use? for the living fetus/continuing pregnancy or the fetus that has passed?
Continuing pregnancy after intrauterine death of one fetus or more, third trimester, fetus 2 (O31.23X2) because fetus 2 is the intrauterine death?
or
Continuing pregnancy after intrauterine death of one fetus or more, third trimester, fetus 1 (O31.23X1) because fetus 1 is alive and being assessed on this DOS?

Use the code that represents the care given and to whom at the time of the visit. If care is directed at the fetus 1, use that code only. You would report fetus 2 for the visit at which that fetus was being assessed.
