Wiki Coordination of care

Looking for coding for coordination of care between family member and physician. Physician spent 30 minutes with patient's family to discuss - diagnosis, impressions, prognosis and recommended diagnostic studies and treatment options. Patient has dx V65.19 other person consulting on behalf of another plus 4 other medical dx. Do we bill 99214 or something other??

If this encounter with the family member was on the same date as another E/M level by the same physician, you can use the prolonged services codes without direct patient contact (99358,99359). These don't usually get paid by insurance, however.
I'm not sure that you would want to use the 99214 if you don't have the physician seeing the patient at all, and I don't know if there is a stand-alone code for non face-to-face time.
Anyone else?
