Do I use 496 or V12.69 when COPD is listed as PMH? (inpatient and outpatient)
 
I believe you would use 496. V12.69 is history of and once you have copd there's no going back. Hope that helps:)
 
Remember when coding from pmh that the condition is to be addressed somehow during the visit (such as med list, lungs on exam, etc.) in order to be valid during an audit. While it is true that COPD will always have a 496, if it isn't addressed during the visit, you should not include it in the dx code list.
