Correct coding of perforated esophogus repair caused by bulemia?

Can anyone help with the ICD-9 diagnosis coding for a perforated esophogus repair caused by bulemia. Since the Behavioral Health managed care organization pays for mental illness primary diagnosis, and medical fee for service pays for medical primary diagnosis, this has become controversial. The same issue for anorexia nervousa related medical problems (excluding malnutrition..I know that one!). Thanks!!
