I have a question regarding the correct use of DM combination codes.
Scenario: Assessment/Plan dx listed are: R42 and E11.9
Problem list also shows pt has history of CKD
Patient not being seen on DOS for the CKD

Do you code E11.9 or do you code E11.22

Can anyone send me documentation as to the correct coding?

If the CKD is only mentioned in the problem list, I would not code E11.22. You need something to support that the CKD is current.
