When getting a denial from BCBS requesting documentation for the E/M, does anyone submit these back as a corrected claim with the frequency code 7 and original DCN or no?
 
Why would you submit a corrected claim just because you are getting a medical records request from the insurance company? I work for an insurance company and if you start submitting corrected claims for all of the E&M claims that you receive a medical records request for this is going to be a red flag to the insurance company.

That isn't to say sometimes it isn't necessary to submit a corrected claim upon review of the claim and the records request, but it shouldn't be the standard practice in response to a medical records request.
