Wiki Correcting ICD9 in previous progress notes

It has come to my attention that there was an incorrect ICD9 descriptor in our database for dysphagia, unspecified. When the doc types in "dysphagia" in the EMR search function, it was coming back with 438.82 at the top of the list, which is a dysphagia, late effect of a cerebral vascular incident. NOT what should have been coded on our patients.

I've run a list of all patients coded with this late effect assignment. What is the correct and legal procedure to correcting thier notes in the EMR to reflect accurate diagnosis?
