Wiki Cortisone Injections Depo Medrol

Can anyone help me with how they are coding for the cortisone injections with depo medrol ? We are using code 20553 with the medication and NDC #. He does for injections into the joints in hands and also the shoulder. I am having a issue getting paid so has the injection code I am using changed or does anyone know of a better code. Also, are you billing the E&M code with the modifier 25?

Joint injections are not reported with 20553.

Basically speaking an E/M code would only be reported when the decision to perform the injections were made during that visit, or another identifiable condition was treated. (You opened a can of worms with that one).
 
Major joints (20610) are shoulder, hip, knee, subacrominal bursa
Intermediate joints (20605) are wrist, ankle, elbow, acromioclavicular
Small joints (20600) are fingers, toes, bursa

Alot of insurances won't pay for an E&M with a joint injection. Doctor needs to eat the E&M in those cases, or do an evaluation and schedule the injection for another day.
