What procedure code should be billed to reimburse the physician for reviewing lab results and adjusting the Coumadin or Warfarin dosage for a patient?
 
If the provider does not see the patient face to face for the discussion then there is no charge to bill. If he does see the patient, then the charges are 99363/99364 every 90 days.
