Can you give me some guidelines how to document for visits based on time?
I would appreciate any help.
 
These are the guidelines I follow:

1. Physician documents total time
2. Documentation states that greater than 50% of the total face to face time was spent on counseling or coordinating the patients care
3. The content of discussion is sufficiently documented
