How to count for bullets using 97 guidelines?
if the provider wrote down PEERLA is this counted as one bullet, what if the provider put it this way "Pupils are equal, round and reactive to light" should we count this as one bullet or 3 bullets instead?
How about Lymphatics: If the lymphs are checked with Neck, Cervical and Groin, we count them as 3, If only 2 were checked we count them as zero. If it is only "No Lymphadenopathy" we count it as 1. is this correct?
