I'm having trouble figuring out the difference between these two mods. Can a provider bill an E/M code twice, once with GT and once with 95. That seem inaccurate to me.

For the same patient on the same DOS? Not with either of those modifiers for sure.
GT and 95 are practically identical. -95 is a newer code and is used by most payers (including Medicare), but some payers still use -GT. Also, you may need to change the POS for the claim, depending on what the payer wants. Some are saying the modifier is enough, some want just the POS changed, some want both.
I recommend putting together a quick cheat for your largest payers. Maybe someone out there in AAPC Land already made one and might want to share it?
