Can Z11.59 be a primary dx? Everything I have read suggests that it can in fact be primary. I got a rejection from BCBS stating service not payable for screening service.
Do you use the Z20.828 even if no symptoms or no known exposure documented?^ Yes what Thomas said, as soon as this guidance was released we started charging them all as Z20.828 and have not had issues with Medicare denials since.
