I have an insurance carrier denying our ASC facility charge saying we should use revenue code 360 ( Operating Room services) but we have always used revenue code 490( Ambulatory Surgical Care) that includes all of our facility fee services. My understanding for revenue code 360, that this is used by hospitals not ASC facilities. I would like to find supporting documentation to use for appealing this with the insurance carrier. Any help would be most appreciative.:)
