A piece of Orthoplast or Aquaplast material is cut from a bolt and heated and fashioned as a splint, or in this case a pad for the shoulder to go under football pads, so the kid can play football. How would one go about billing for this? Any supply code is going to require a copy of the invoice, correct? We are looking at A4590, A4570, 99070, or even 97760. Any help would be appreciated.
