Can anyone tell me where i can find docs on billing the 99215 to mericare when a pt comes in for a cpe?

Help please...i'm trying to get things together for a meeting tom. And i need supported documentation.

And also the words "med refill" in the cc section...

I want to make sure I'm clear on your question. Are you saying that your physicians bill for a 99215 when a patient comes in for a CPE (complete physical exam)? What is the actual scenario? Are they billing a problem oriented diagnosis in order to receive reimbursement?
