Medicare is denying 11981 (insertion drug-delivery implant) being used to deliver anti-biotics to patients after surgery. They denied as COB7 (provider not elig on DOS). the provider is eligible and Medicare paid other codes billed with this one. Any ideas why we can't get paid for this code? Thanks!
