Hi, all. Not sure how it happened, but there are a few pages missing from my 2013 CPT book (OK, I have some ideas, but don't want to start trouble, just want to get the info I need ASAP).

Does anyone have a book they would be willing to fax me a couple pages? Thanks!!!
