United Healthcare denied a claim billed with CPT 29581-GP-RT-XS and 97597-GP and 97598-GP, stating "procedure inconsistent with the modifier used or a required modifier is missing". I have searched United Healthcare billing and coding guidelines as well as their modifier policy and outpatient physical therapy policy, and I cannot find any specific information regarding modifiers with CPT codes 29581 and/or 97597 and 97598. If anyone has had a similar issue and managed to find a solution, I would appreciate if you could share your findings or possible send me a link to the appropriate guidelines.
