Wiki CPT 30140 being paid at the facility rate

When we perform BITS (30140) in the office all the insurance companies pay this procedure at the facility rate and not the non-facility rate. Is this happening to anyone else? Does anyone know why they would pay that code at a facility rate even when it is done in the office/POS 11?
Any and all help is greatly appreciated.
