Wiki CPT 33268 for prevention

Can someone help me with the new CPT 33268? I have a provider that is not telling me the patient has a history or diagnosis of afib nor is he providing a CHADs score. On STS it clearly states that the LAA clip should not be billed if used for prevention, but it has the old method for billing for the Atriclip. Can someone tell me if this is medically necessary if no Afib diagnosis? Thanks.
