I have cpt 59510 with dx 669.71,656.61,664.11,v27.0 . denied by bcbs as dx is inconsitence with Px. I need help regarding this.

Use the 656.61 Large for dates as primary as that is probably why the section was performed. Your doc didn't give any indication as to why? This wasn't a repeat or Obstruction caused by size of infant? I never use unspecified codes unless there isn't any possible reason for it. since you also coded a laceration 664.11, was it failed trial of labor?
