Is anyone else finding that UHC plans have begun changing bilateral 67311 (1 horizontal muscle each eye) codes to 67312 and processing as such? They appear to be interpreting 67312 as 2 horizontal muscles total as opposed to 2 horizontal muscles in 1 eye. And, if so, is anyone having luck appealing the denial? Or has the definition of 67312 been changed?
