Hi All-

One of our docs is now billing 68040 and we cannot find anywhere as to whether or not we can bill per eyelid or bilaterally or unilaterally only. Does anyone have any insight on this? Thanks!
 
It is per eye with a zero day global. Use of the operating microscope would not be reported separately. This from both the Ophthalmology Coding Companion from Optum and the Cornea Coding Module from the American Academy of Ophthalmic Executives.
