We are a family practice and recently we have been getting denials for CPT code 81003-QW (DUA) stating that this code is a duplicate and has been furnished by another provider. The other provider is Quest laboratory and this is happening when a provider orders a urinalysis with reflex to culture after the DUA is done in-house and further testing needs to be done. This was never a problem but we are coming across multiple denials now from different insurance companies. I found a modifier 91 to possibly use, but it does state not to use if it is ordered to confirm results. I was wondering if anyone else is having this issue as well? Any suggestions?
