Wiki cpt 81003 with E & M

do we need to add modifier 25 on E & M code when bill with 81003? and does 81003 need QW modifier? please help

you do not need a modifier 25.
QW would be appropriate if your practice/provider has a CLIA certificate of waiver and the test was performed in clinic.
