I have gotten several denial lately from Cigna Medicare Advantage. I called today and was told this CPT is not payable because of LCD? I've never heard of that before. Is anybody having trouble with the same issue? TIA

Did you try searching for your Fiscal Intermediary's LCD to see if there are any restrictions or limitations on coverage?
