I recently had a few denials due to ICD code which had been payable previously. I am in Ohio and can not find am LCD or changes to the CMS guidelines on this code. Anyone else having issues? For this particular patient I have billed Z95.810 and I50.22, which has been billed multiple times and paid in the past. I know some MACs do not want the Z code but I do not see anything mentioned in my MAC.
