Does anyone know if these codes can ONLY be submitted by a Dietician or Nutritionist? I can't seem to find anything stating these can't be used by a physician or NP...

There's a footnote in CPT:

"(For medical nutrition therapy assessment and/or intervention performed by a physician, see Evaluation and Management or Preventive Medicine service codes)"
