We saw a patient that has pyoderma around her colostomy. The MD injected the area with Kenalog/Lidocaine. I billed 11900. I also billed 99214 w/modifier 25. Medicare "Your claim has been separated to expedite handling. On the EOB for 11900 - Incomplete/Invalid procedure modifier(s). This is the first time I am trying to code this type of claim - any suggestions?
