Can anyone please tell me where I can find a example surgery report when using cpt code 36800? I understand that this procedure involves vein to vein. Any help would be appreciated . Thank you

I am not sure this will be helpful. It is a small component of the central venous line; we have it listed as a cannulation of external fistula. According to my doc it is somewhat obsolete.
 
thank you but I was looking for a report to see how it would be written to do that procedure.
