Hi all,
A patient came into the office for this test. 82274 blood, occult by fecal hemoglobin determination by immunoassay , qualitative, feces 1-3 simultaneous determinations.

I billed this with appropriate E&M code.

I added modifier QW (CLIA waived) and medicare denied.

I did a resubmit on this with modifier 25 on office visit (dr. has not done these before so I am unfamiliar) and it denied again.

The medicare denial is 16 claim/service lacks information or has submission billing errors.

Can anyone tell me what I am not doing correctly.

Was this test done during a regular E&M visit or is this for the annual physical? What DX are you using to support the necessity of the test?
