When a patient has 2 individual sessions in one day with the cpt code 90837 and the insurance can't pay out the second one, and also 2 patients have 2 individual sessions on the same day but with 2 different providers and sometimes with the same provider with an extended session cpt code added.

What would be the appropriate coding for this?
