Hello All !!! I need some help please-I have a new company Im working for has outsourced billing that does not put the provider in box 17 on form 1500, I've never seen that before and found it odd. This company states the claims process this way any one ever seen this before.

Another question is they are using POS 10 for CPT Code 99345 everything I have researched points to POS 12. Any suggestions?
