Wiki cpt codes 91040 and 43249

My doctor is performing an EGD and does a balloon dilation of the esophagus, a biopsy of the stomach and then at the bottom of the note says that he performed an endoflip with all the measurements and his interpretation. Can you bill 43249, 43239 and 91040,26 together? This is for the professional fees not the facility.
I can't find anything saying that 43249 and 91040 can't be billed together, but my claims are not paying for 91040 because it says it is bundled.
Any help would be appreciated

Per NCCI edit, CPT 43249/91040 are not allowed at all. More than likely your just going to code CPT 91040.26, CPT 43239 is ok.
