I'm just starting cardiology coding and am looking for some help with this chart.
"Patient became hyperkalemic (K=5.8) with PVCs and left femoral dialysis line not functional. Decision was made for emergent right IJ line placement. The prior swan ganz was removed and wire placed through cordis. Next the cordis catheter was removed over wire and dialysis catheter placed over wire via Seldinger technique. All ports easily flushed. Next catheter sutured in place and CXR ordered."
