What's the place of service? Outpatient hospital, or freestanding? If freestanding, do you bill separately for the TC & PC? Or do you bill freestanding globally on one claim?

Also, are you billing for the physician or the facility?

We are billing the charges under POS 11 and billing for the physician. CPT 77014 was usually billed globally but for HMO) billed with splitting as TC & 26.

You would not report G6015 on a physician split claim. That's the treatment delivery charge, not a physician charge.

If you're split billing the TC/PC in a freestanding setting, it should go on the office charge. The physician would only bill the IGRT with a 26 modifier. The office would bill the G6015 and the IGRT with a TC modifier.
