Does anyone know of any publications/books/whatever to help me understand what all is included in a procedure from prep to recovery as well as usual times to perform the procedure?

We are having disagreements on what is included or not and either should be coded additionally or added as points to the E/M for our ER facility coding.

Any help would be greatly appreciated.
